to set up the actual Pay Incentive Request. Once both Pages 1, 2, & 3 are completed and submitted, with the attachments, your "Pay Incentive" will be processed in 3-5 business days! 

Step #3: Congrats, You're Now Enrolled!!

Click "Paycare Promise" Page 3" to complete your membership.  We can assist multiple times in your 1-year membership period! *Be sure you've completed #1 & #2 above, and remember to also send the required attachments! Now, you're well on your way to receiving your first "Pay Incentive"!
